Inpatient Care Manager – BSN

The nursing manager acts as a registered undergraduate level nursing manager responsible for evaluating, coordinating patient care, evaluating and updating the care plan across the continuum using recognized criteria and guidelines to manage and determine the medical necessity of the services provided. The RN Care Manager acts as a clinician, case manager and trainer to achieve optimal clinical and qualitative results by effectively managing care and resources to reduce unnecessary workload. Contributes to the development and implementation of performance improvement activities.
Minimum requirements: BSN required.
Three to five years of experience in one of the following settings: case management, disease management, home care, hospital, or doctor’s office.
Ability to work independently and set priorities to efficiently coordinate the care plan. Acknowledge the role of cultural, social, economic, and behavioral factors in the accessibility, availability, acceptance, and delivery of public health services, and respond to diverse needs that arise from these differences. Incorporate strategies for interacting with people from different backgrounds (e.g., cultural, socio-economic, educational, racial, gender, age, ethnic, belonging, mental, and physical skills). Proven leadership skills. Effective communication skills (oral and written). Excellent customer service skills. Effective behavioral and educational strategies, including, but not limited to, motivational interviews, teach-back methods, and self-management support.
Preferred qualifications: MSN preferred. Bilingual preferred if defined population requires.
License and Certifications: PA Registered Professional Nurse License. BSN. Case Management certification within 3 years of hiring.
